How to Submit Your Biometric Screening Form for Incentive Processing? Follow
This form may be used when there is a biometric screening incentive. Quest, LabCorp, and other Cigna contracted national and regional lab partners may send labs and biometrics. There are various ways you can satisfy the Biometrics activity, in most cases a form would not be needed. Please contact your service team for any client specific information.
There are two ways to submit your Biometric Screening form for processing:
- via the ICR FAX number 800-882-8574
- through the Rewards page
This form can be located on your Wellness account, both on the website and app versions.
The navigation steps listed below will depict a side-by-side comparison of both the App and Web versions navigation route.
- From your Home page, navigate to Explore Wellness & Rewards.
- Next, select Rewards to access your rewards page.
On your Rewards page, locate the Biometric Screening activity and click on the corresponding link. It will be located under Preventive Care Initiative.
Note: The reward amount may vary depending on your specific plan.
- After clicking on the Biometric Screening activity, a new tab will open with options to download and submit your biometric screening form.
- Firstly, choose the option to Download the form in order to print it and have your measurements filled in by a healthcare professional.
After obtaining the form, there are some things you should be careful with:
- Folds, wrinkles, or notes on the form may cause the form to not process or error out.
- Please be sure to write clearly, sign and date the form properly. The form cannot be processed if incomplete or unreadable.
Estimated timing for the form to process
For the Biometric form, it can take up to 30 business days to process. Biometrics will be visible on the Stats page when processed and if rewards are available for biometrics those will be displayed on the Rewards page.
For the Reasonable Alternative and Waiver Form - can take up to 30 business days for the form to process once it is uploaded, faxed, or received by mail.
Note: You can expect to receive an email upon receipt of your form and after the form has completed processing. If the form is missing information an additional outreach email may also be received.
