How to access the Wellness Questionnaire Follow
To access your Wellness Questionnaire, log in to your account on the Evernorth website and navigate to the Home page.
- Click on Wellness > Health
- Scroll down a bit and click on Wellness Questionnaire
- The Wellness Questionnaire will open-up in a new tab
- Click on Start
You will be redirected to page from where you can start your Wellness Questionnaire by clicking on Start The Survey.
The Wellness Questionnaire survey allows you to re-take it at any point for as many times as you like. Still and all, please keep in mind that you will only be rewarded once per program for this specific activity.
